进展性脑卒中危险因素的临床分析

摘要
目的探讨进展性脑卒中发病的相关危险因素及预测强度。方法收集脑卒中患者234例,其中进展性脑卒中(SIP)患者116例,非进展性脑卒中(NSIP)患者118例。详细记录两组患者的高血压病史、糖尿病史、颈动脉狭窄、入院血糖、血浆同型半胱氨酸(Hcy)浓度以及次日空腹血脂情况,包括甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)和低密度脂蛋白胆固醇(LDL-C)值。结果两组病例高血压病史、糖尿病史、颈动脉狭窄以及入院次日空腹血糖、Hcy值、空腹TC、TG、LDL-C比较差异有统计学意义(P<0.05),Logistic回归分析结果显示高血压病史、空腹血糖、颈动脉狭窄、hs-CRP和Hcy等指标是SIP的独立危险因素。结论 SIP的发病是多因素、多机制共同作用的过程,既往高血压和发生脑卒中后骤然降压、持续高血糖、大动脉硬化及狭窄、Hcy以及高血脂、高凝状态等构成了缺血性脑卒中进展的主要因素。
