儿茶酚胺氧位甲基转移酶基因多态性与疼痛的相关性研究

摘要
疼痛感觉受环境和遗传因素影响,个体对疼痛的敏感性与遗传因素有关。已发现多种基因变异与疼痛相关。儿茶酚胺氧位甲基转移酶(catechol-O-methyltransferase,COMT)主要功能是代谢具有毒性或生物活性的儿茶酚胺类物质及降解外周系统的多巴胺。目前COMT基因有293个单核苷酸多态,研究最多最有意义的基因多态性是第4号外显子1947位(rs 4680)上G→A,密码子108/158位缬氨酸变为蛋氨酸。COMT有3个最常见的单倍体型:低疼痛敏感型单倍体型(LPS)、中疼痛敏感型单倍体型(APS)、高疼痛敏感型单倍体型(HPS)[1]。APS/HPS二倍体型杂合子的疼痛阈值和耐受力最高,APS/APS二倍体型纯合子中等,LPS/LPS二倍体型纯合子最低。COMT多态性与多种疼痛相关,如骨骼肌疼痛、纤维肌痛症、术后痛、头痛及神经性疼痛,但结果常存在差异,需深入研究。
