结直肠癌流行病学东西方差异对肿瘤部位的影响

既往认为亚洲结直肠癌的发病率比西方国家低,但目前在亚洲人群中结直肠癌不仅高发,且发病率呈上升趋势,其中直肠癌占绝大部分。生活方式特别是饮食习惯西方化可能是导致结肠癌发病率迅速上升的重要原因,而相比之下直肠癌的发病率比较平稳。结肠癌与直肠癌病因不同,远端结肠和直肠更多与环境因素、水资源污染、饮酒、吸烟有关,直肠癌发病率变化不大可能与其受生活方式影响较小有关。值得注意的是,尽管西方国家结直肠癌的发病率总体处于下降趋势,但年轻人群结直肠癌发病率不明原因地不断上升,特别是直肠癌。从治疗方面看,亚洲与西方治疗方式略有不同,但在过去10年世界范围直肠癌患者生存率均提高。而筛查能有效降低结直肠癌发病率,提高生存率。
