卒中后失语患者脑功能成像与失语商的相关性

摘要
目的通过研究失语症患者静息态脑活动的低频振幅变化及其与失语商的相关性, 以探讨失语症的发生及恢复机制。方法采用低频振幅(amplitude of low frequency fluctuation, ALFF)算法的血氧水平依赖功能磁共振成像( blood oxygenation level dependent functional MRI, BOLD-fMRI)技术, 运用西门子3.0T磁共振仪对12例卒中后失语患者和20例正常对照者进行扫描获得静息态数据, 采用DPARSF软件对静息态数据进行预处理, 然后运用低频振幅算法对数据进行分析, REST软件行两样本t检验, 并对失语组ALFF减低的脑区与失语商做相关分析。结果失语组ALFF减低脑区有左侧颞中回、左侧前额叶内侧回和右侧小脑。3个脑区失语商呈正相关, 相关系数分别为rRoi1=0.48, rRoi2=0.36, rRoi3=0.28。失语组ALFF高于正常对照组的脑区有左侧枕叶、中央前回、岛叶及右侧楔前叶。结论失语组ALFF明显减低且与失语商呈正相关的脑区, 可能是失语发生机制之一;失语组ALFF明显增高的脑区, 可能参与语言功能的恢复。
