大鼠切口痛模型中右美托咪定对瑞芬太尼诱发的痛觉过敏的影响

摘要
目的研究大鼠切口痛模型中右美托咪定(DEX)对瑞芬太尼诱发的痛觉过敏的影响。方法60只雄性SD大鼠随机均分成5组:对照组(C),切口痛组(I),切口痛+瑞芬太尼组(R),切口痛+DEX组(ID),切口痛+瑞芬太尼十DEX组(RD)。ID、RD组于七氟醚麻醉前10min腹部皮下注射DEX50μg/kg。除C组外均需制作切口痛模型。切口痛制作方法:右后爪跖肌位置纵行切开1 cm长的皮肤和筋膜,皮肤行褥式缝合。各组均测定并计算出术前24h(T0),术后6h(T1),24h(T2),48h(T3)大鼠右后爪的机械缩足反射阈值(PMW)及热缩足反射阈值(PWTL)。结果在T1、T2、T3时点,与I组[(7.70±0.67)g,(10.79±1.83)g,(10.97±1.87)g]相比,R组[(3.72±0.71)g,(6.54±1.27)g,(7.27±1.53)g]的PMW明显降低(P<0.05);与I组[(20.74±2.72)s,(16.56±1.38)s,(22.55±2.01)s]相比,R组[(11.81±2.52)s,(11.27±2.30)s,(12.30±2.21)s]的PWTL明显降低(P<0.05)。与I组相比,ID组只能缓解T1时点PMW的降低(P<0.05)。与R组相比,RD组可以缓解T1[(14.03±1.01)g]、T2[(14.27±1.33)g]、T3(14.26±1.58)g]时点的PMW值的降低(P<0.05)及T1[(23.56±1.53)s]、T2[(16.54±3.24)s]时点的PWTL值的降低(P<0.05)。结论大鼠切口痛模型中,瑞芬太尼可以诱发切口周围组织的痛觉过敏;右美托咪定可以预防瑞芬太尼诱发的痛觉过敏。
