307例放射性脑脊髓病分析

摘要：

放射治疗邻近中枢神经系统的恶性肿瘤16,921例,并发放射性脑脊髓病(IMED)307例。发生率为1.8%。分四型:一过型、静止型、渐进型和急进型。低头有触电感是典型的一过型,如不注意休息和治疗,有可能发展。在101例一过型病例中,有15例发展为急进型,最终致死。按损伤部位分:(1)大脑组,(2)脑干和颈段脊髓组,(3)胸段脊髓组。预防IMED,照射技术是一重要因素.射野越多,越易产生深层组织剂量重叠,为此建议:在照射头颈部肿瘤时,应先用面颈联合野,4,000rad后,改用多野、小野照射。治疗IMED,一般用大量维生素、激素和脱水剂,并让患者休息。
