摘要：

皮质醇激素在脓毒症治疗中的应用已有较长历史。20世纪50年代的临床研究肯定了大剂量激素在脓毒症治疗中的疗效,但美国感染协会已于1992年基本否定了激素作为常规治疗方案在严重脓毒症患者中的应用。而后小剂量激素被认为可明显改善脓毒症患者预后而获得推荐。2008年,大规模的欧洲多中心临床试验(CORTICUS)报告却显示小剂量激素治疗并不能降低脓毒症患者的病死率。研究者对激素治疗对象的选择、肾上腺功能相对不足的标准以及疗程的长短等问题仍有争议。因此,脓毒症激素治疗的方式及疗效还有待进一步探讨。
