中颅窝脑外海绵状血管瘤

摘要：

本文报告颅内海绵状血管瘤(CA)44例,占同期脑血管畸形的4.6%。位于中颅窝者34例,后者有如下特点:(1)女性多见(男女之比为1∶4.6);(2)多在中年发病,平均就诊年龄为45岁;(3)同期颅内 CA 的脑外与脑内之比为1∶3.4;(4)症状除头痛外,眼部征象(动眼神经与外展神经损害,视力减退及突眼等)较常见;(5)CT、MRI 及数字减影血管造影(DSA)有其特点,但术前与中颅窝脑膜瘤之鉴别困难;(6)肿瘤血运丰富,术前备血要充分,术中囊内切除如出血太多,可填充明胶海绵后缝合中颅窝底之硬膜;(7)放疗可使肿瘤皱缩,手术加放疗可获较长的生存期。
