冠状动脉旁路术后主动脉内气囊反搏的应用

摘要：

冠状动脉旁路移植术患者 (CABG) 5 0 6例 ,19例使用主动脉内气囊反搏 (IABP)。CPB减流量时若循环不能维持正常 ,加大正性肌力药物用量、延长体外循环辅助时间后仍不能停机 ,立即经股动脉放置IABP。IABP占全部CABG的 2 4 % ,占CPB下CABG的6 4 % ,非体外循环下搭桥未使用IABP。IABP辅助时间 (2 5 3±5 6 )h,16例顺利渡过低心排阶段 ,2例因围手术期心肌梗死、1例因鱼精蛋白过敏性休克死亡 ,死亡率 15 8%。 1例右髂总动脉穿破 ,另 1例出现右足背动脉栓塞 ,并发症发生率 10 5 %。作者认为 ,出现心功能不全时应尽早使用IABP ,其效果好 ,并发症少 ,但发生围手术期心肌梗死后使用效果差。
