老年急性心梗患者QRS时限异常与室性心律失常发生率的相关性

摘要：

目的探讨老年急性心肌梗死(AMI)患者的QRS波时限异常与室性心律失常(VA)的概率的相关性。方法参照12导联心电图QRS波群时限将AMI老年患者114例分成A组(>160 ms)36例,B组(120～160 ms)45例,C组(<120 ms)33例。比较3组基线资料、心率变异性(HRV)及VA发生结果。结果 3组性别、年龄、体质指数(BMI)、高血压史、糖尿病史等均无统计学差异(P>0.05);3组Ⅲ级(χ2=7.903,P=0.019)、ⅣB级(χ2=8.962,P=0.011)VA发生率有统计学差异,且均为A组发生率最高,C组最低;3组HRV指标包括24 h正常R-R间期标准差(SDNN)、连续5 min 24 h正常R-R间期标准差(SDANN)、相邻正常R-R间期标准差(RMSSD)、超过50 ms百分比的相邻正常R-R间期差值〔p NN50(%)〕和三角指数均有显著差异(P<0.01),且均为A组最低,C组最高。结论老年AMI患者QRS时限异常与VA的发生率有明显相关性。
