<正> 肝纤维化(liver fibrosis)是临床上常见的病理改变,是各种慢性肝病产生的一种共同反应,可视为肝硬变的早期阶段。其病理改变的特点为:1.细胞外基质(ECM)在肝脏湿重中所占的比例增加,由正常时不到0.6%升高3~6倍;2.ECM各种成分不成比例地升高,如各型胶原中以Ⅰ、Ⅳ及Ⅲ型胶原升高为主,蛋白多糖以硫酸皮肤素(DS)及硫软骨素(CS)升高为主,结构糖蛋白以纤维连接蛋白(FN)及层粘连蛋白(LN)等升高为主;3.一些基质分子的微细结构发生变化,如胶原d链的羟化程度及糖胺聚糖(GAG)的硫酸化程度发生改变等;4.ECM在肝组织中出现局部性再分布,表现为ECM主要沉积于汇管区及窦周间隙(Disse隙),并且出现窦周毛细血管化(capillarization)。目前认为,肝纤维化时肝组织中ECM的病理性改变是由于控制ECM形成及降解的稳
