脑白质病变对阿尔茨海默病锥体外系体征的影响

摘要：

目的探讨脑白质病变(WMLs)对阿尔茨海默病(AD)患者锥体外系体征(EPSs)的影响。方法临床收集轻度AD患者77例,根据MRIT2加权像是否合并WMLs分为单纯AD组34例和AD合并WMLs组43例,采用简明精神状态量表(MMSE)、统一帕金森病评定量表(UPDRS)运动部分和日常生活活动量表(ADL)分别进行认知功能、EPSs和日常生活能力的评估。结果与单纯AD组相比,AD合并WMLs组ADL评分、UPDRS运动部分总评分以及强直、运动迟缓、轴性损害单项评分均显著升高(P<0.05);控制了年龄、血管危险因素和认知功能的影响后,运动迟缓、轴性损害与AD合并WMLs仍具有相关性。结论WMLs可促进AD患者EPSs的恶化,积极预防WMLs有助于减轻AD患者的EPSs。
