溃疡性结肠炎相关大肠癌研究进展

摘要：

溃疡性结肠炎相关大肠癌指的是在溃疡性结肠炎(ulcerative colitis,UC)背景下发生的大肠癌,UC患者大肠癌的发生率明显高于一般人群,而且随病程的延长而上升。慢性炎症刺激通过氧化应激造成肠黏膜损伤,引起细胞增殖调控的相关基因的突变和表观遗传改变,从而导致结肠上皮细胞增殖和转运加速,最终可发生异型增生和癌变。对长期UC患者必须进行大肠镜监测,若在炎症区域发现异型增生的息肉或腺瘤样病变可将其完整切除,非腺瘤样重度异型增生和广泛的异型增生病变则应考虑手术切除结肠。5-氨基水杨酸类药物通过减轻炎症可预防癌变发生。
