重复经颅磁刺激治疗对卒中后偏瘫肩痛患者上肢运动功能及日常活动能力的影响

摘要
目的探讨经颅磁刺激治疗对脑卒中后偏瘫肩痛患者上肢运动功能、生活质量以及疼痛程度的临床疗效。方法选取脑卒中后偏瘫伴肩痛患者100例,按随机数字表法分为观察组(n=54)和对照组(n=46)。对照组患者采用常规康复治疗及理疗,观察组在此基础上增加经颅磁刺激治疗。两组患者均于治疗前和治疗后5周分别进行Fugl-Meyer运动功能评定量表(FMA)、世界卫生组织生活质量评定简表(WHOQOL)和肩关节疼痛目测类比法(VAS)评定。结果两组患者性别、年龄、病程等一般资料以及治疗前FMA、WHOQOL、肩关节疼痛VAS评分比较组间差异无统计学意义(P> 0. 05)。分别经5周治疗后,两组肩关节疼痛VAS评分均较治疗前明显降低(P <0. 05),且观察组治疗后疼痛VAS评分的改善较对照组更显著(P <0. 05)。对照组治疗后FMA、WHOQOL较治疗前无明显改善(P> 0. 05),而观察组该两项指标均较治疗前改善明显(P <0. 05)。结论经颅磁刺激治疗可显著改善脑卒中后偏瘫伴肩痛患者的上肢运动功能、生活质量和疼痛程度,其效果优于常规康复治疗。
