27例围手术期主动脉内球囊反搏治疗的分析

摘要：

目的　报道 2 7例围手术期应用主动脉球囊反搏 ( IABP)治疗心脏术前、术中、术后顽固性心功能不全和心绞痛经验 ,评估 IABP的有效性、死亡率和并发症发生率。方法　自 1999年 3月至 2 0 0 0年 11月在协和医院连续 2 7例患者围手术期应用 IABP治疗 ,其中心脏瓣膜疾病 10例、冠心病 14例、扩张性心肌病 1例、左房粘液瘤 1例、肾肿瘤1例。结果　 2 0例有效 ( 74.0 % ) ;7例死亡 ( 2 6.0 % )。IABP致命性并发症发生情况 ,下肢缺血表现 1例 ( 3 .7% ) ;局部血肿 4例 ( 14.8% ) ;神经系统症状 2例 ( 7.4% )。结论　 IABP对围手术期药物治疗无效的因心肌供血不足而引起的顽固性心功能不全和心绞痛有明显作用。穿刺操作熟练可降低并发症发生 ,严格掌握适应证范围有助于降低死亡率
