脓毒血症患者血清白细胞介素-27和C-反应蛋白及降钙素原水平与病情严重程度的关系研究

摘要
目的探讨脓毒血症患者血清白细胞介素-27(IL-27)、C-反应蛋白(CRP)和降钙素原(PCT)水平,及血清IL-27、CRP、PCT与脓毒血症预后的关系。方法选择2013年1月-2015年12月医院收治的脓毒血症患者80例作为脓毒血症组,全身炎症反应综合征非脓毒血症患者80例作为对照组;观察分析两组患者血清IL-27、CRP、PCT水平。结果脓毒血症组患者血清IL-27和PCT水平高于对照组(P<0.05);IL-27诊断脓毒血症的灵敏度、特异度、阳性预测值、阴性预测值、ROC曲线下面积分别为67.58%、73.12%、62.13%、76.52%、0.753;CRP诊断脓毒血症的灵敏度、特异度、阳性预测值、阴性预测值、ROC曲线下面积分别为54.23%、64.35%、69.87%、53.24%、0.594;PCT诊断脓毒血症的灵敏度、特异度、阳性预测值、阴性预测值、ROC曲线下面积分别为79.03%、76.42%、79.58%、57.41%、0.845;脓毒血症休克患者血清IL-27和PCT水平高于脓毒血症非休克患者(P<0.05);脓毒血症死亡患者血清IL-27和PCT水平高于脓毒血症存活患者(P<0.05)。结论脓毒血症患者血清IL-27和PCT水平升高,IL-27和PCT在脓毒血症的诊断、严重程度及预后评估中具有一定价值。
