冠状动脉介入术病人出院后遵医行为的随访分析

摘要：

[目的]探讨冠状动脉介入治疗病人出院后遵医行为和预后的关系。[方法]选择2010年1月—2010年6月北京安贞医院心内科经皮冠状动脉介入（PCI）治疗的住院病人158例,出院后6个月12个月由专职护士进行电话随访,记录遵医行为以及复发胸痛、支架内再狭窄等临床终点。[结果]158例病人经电话随访失访28例（17.7%）,其余130例病人年龄为57.5岁±9.8岁;遵医行为分值复发胸痛组5.48分±1.08分较无复发胸痛组6.83分±0.38分低（P=0.000）,支架内再狭窄组5.20分±1.37分较无支架内再狭窄组6.28分±0.94分低（P=0.009）;并且随分值降低,病人复发胸痛（OR为21.102,95%CI为8.005～55.628,P=0.000）及支架内再狭窄（OR为2.183,95%CI为1.377～3.462,P=0.001）风险均增高。[结论]冠心病PCI治疗病人出院后的遵医行为有助于识别术后复发胸痛及支架内再狭窄的高危病人,对强化冠心病病人二级预防治疗和改善病人预后有益。
