老年高血压患者脑卒中发生情况与血压控制状况的相关性

摘要：

目的分析老年人群中高血压患者的血压控制情况与脑卒中发生情况的相关性。方法选择2 373例5年定期体检患者,查阅保健病历,结合动态血压报告,参考家庭测量数据,统计分析脑卒中发生与血压控制情况的相关性,分析患者降压治疗中各影响因素。结果血压控制良好者脑卒中发生率最低,而血压控制不佳患者(血压控制长期不达标及血压间断达标但波动较大)脑卒中发生率最高,与控制良好组比较差异显著(P<0.05)。血压控制不良组脑卒中发生率与血压波动组比较无差异(P=0.417);血压较低者脑卒中发生率略高于血压控制良好组,但二者无统计学差异(P=0.155)。结论单就预防脑卒中发生,老年人血压在可以耐受的情况下宜低不宜高,控制血压宜平稳不宜波动。影响血压控制好坏的因素很多,患者自身依从性以及监护人细致的生活护理极为重要。老年人应加强血压监测,各方面共同努力,予个体化治疗,使血压控制在合理水平,预防脑卒中发生。
