振动结合上肢任务导向性训练对脑卒中后偏瘫患者上肢运动功能的影响

摘要
目的:探讨振动结合上肢任务导向性训练对脑卒中后偏瘫患者上肢运动功能的影响。方法:将74例符合诊断标准的脑卒中患者随机分配为对照组,治疗组1(振动组),治疗组2(振动结合上肢任务导向性训练组)。三组患者均于治疗前和治疗4周后采用简化Fugl-Meyer评分量表(FMA)、Wolf运动功能测试(WMFT)、患侧手最大握力及改良Ashworth量表(MAS)分别进行评定。结果:在训练后,治疗组1比对照组在FMA(44.38±15.29)、WMFT(43.88±14.50)、患侧手的最大握力(21.50±11.68)及MAS(1.00±1.02)有显著改善,此外治疗组2比另外两组在FMA(48.92±13.60)、WMFT(52.92±13.74)、相关手的最大握力(32.80±10.61)及MAS(0.62±0.65)有显著改善。结论:在脑卒中患者中,运用振动结合上肢任务导向性训练比单独运用上肢功能训练或振动训练,在上肢功能、痉挛及最大握力的改善方面有更大的获益。
