0.75%罗哌卡因椎管内联合麻醉在剖宫产手术中的效果评价

摘要：

目的 :探讨 0 .75 %罗哌卡因椎管内联合麻醉在剖宫产手术中的安全性和可靠性。方法 :选择 ASA ～级足月妊娠产妇 4 0例 ,随机分为两组 :腰麻用药分别用 0 .75 %罗哌卡因 (R组 )和 0 .75 %布比卡因 (B组 ) ,用量均为 1.2～ 1.5 m l。术中连续监测呼吸和循环状态 ,评估麻醉效果 ,并观察围术期的不良反应 ,评估出生时新生儿 Apgar评分。结果 :两组产妇术中清醒合作 ,麻醉效果均达到手术要求 ,最高痛觉阻滞平面无差异 (P >0 .0 5 ) ,R组起效时间较 B组长 (P <0 .0 1) ,R组感觉和运动神经阻滞起效慢 ,维持时间短 (P <0 .0 5 ) ,两组新生儿娩出时 Apgar评分相似 (P >0 .0 5 )。Bromage评分 :R组明显小于 B组 (P <0 .0 1)。术中 R组和 B组在新生儿娩出 MAP下降明显 (P <0 .0 5 ) ,两组产妇在新生儿娩出时 HR增快 (P >0 .0 5 )。结论 :0 .75 %罗哌卡因椎管内联合麻醉用于剖宫产手术安全有效 ,运动阻滞恢复快
