臭氧关节腔注射对胶原诱导性关节炎大鼠血清细胞因子的影响

摘要
目的观察关节腔内注射臭氧对胶原诱导性关节炎(CIA)大鼠血清中TNF-α、IL-6、IL-17A及血管内皮生长因子(VEGF)的影响, 探讨臭氧治疗RA的作用机制。方法将32只体质量及鼠龄相当的Wistar大鼠采用随机数字表法, 随机分为正常对照组(对照组)、CIA模型组(CIA组)、臭氧组、甲氨蝶呤组。除正常对照组外, 采用注射弗氏完全佐剂+牛Ⅱ型胶原蛋白的方法建立CIA大鼠模型。造模后14 d, 臭氧组双踝关节内注射浓度为40 μg/ml的臭氧各1 ml, 每周1次, 共注射3周。甲氨蝶呤组0.9 mg/kg, 每周1次, 共3周。分别测量各组大鼠双后肢足肿胀度。治疗结束后1周, 用流式多因子检测技术测定各组大鼠血清中TNF-α、IL-6、IL-17A及VEGF的水平。采用单因素方差分析或Kruskl-Wallis秩和检验进行统计分析。结果治疗结束后, 臭氧组大鼠足肿胀度减小, 与模型组相比差异有统计学意义[(4.21±0.14)ml与(9.12±0.17)ml, t=8.43, P<0.05]。与CIA模型组相比, 臭氧组血清TNF-α、IL-6及VEGF浓度明显降低, 差异有统计学意义[91.55(86.55, 98.53)pg/ml与14.45(12.55, 16.15)pg/ml, χ2=6.216, P=0.002;145.08(37.44, 362.82)pg/ml与5.84(5.47, 15.93)pg/ml, χ2=13.136, P=0.004;51.56(46.09, 74.10)pg/ml与36.22(32.18, 41.69)pg/ml, χ2=3.732, P=0.002], 臭氧组血清TNF-α、IL-6、VEGF浓度与甲氨蝶呤组相比, 差异无统计学意义[14.45(12.55, 16.15)pg/ml与(12.45(11.80, 15.60)pg/ml, χ2=0.243, P>0.05;5.84(5.47, 15.93)pg/ml与7.86(5.25, 15.25)pg/ml, χ2=0.058, P>0.05;36.22(32.18, 41.69)pg/ml与40.17(35.47, 50.73)pg/ml, χ2=0.516, P>0.05]。臭氧组与对照组、CIA组、甲氨蝶呤组3组间血清IL-17A水平相当, 差异无统计学意义(F=1.827, P=0.165)。结论关节内注射浓度为40 μg/ml的臭氧可减轻CIA大鼠的关节肿胀度, 作用机制可能与臭氧可以降低血清TNF-α、IL-6及VEGF水平有关。
