211例急性肾功能衰竭的流行病学分析

目的探讨急性肾功能衰竭(acuterenalfailure,ARF)流行病学和预后变化及其相关因素。方法收集北京大学第三医院近10年出院病例中含有ARF诊断的患者的临床资料,重新确认诊断后,分析流行病学、预后及其相关因素变化。结果共搜集并确认ARF211例,占同期出院患者的1·2‰,近5年呈增高趋势。医院获得性ARF在2000年后显著增高(P<0·05),尤其ICU患者的ARF发病率增加1·07倍。患者病亡率高达37·91%,并有继续增高的趋势,其中医院获得性ARF显著高于社区获得性ARF(P<0·01)。结论近10年来,住院患者中ARF比率呈增加趋势,而预后无明显改善,医院获得性ARF发生率增加是主要原因。
