双源CT和冠状动脉造影对冠状动脉斑块评估比较研究

摘要：

目的:使用双源CT对冠状动脉斑块进行无创评估,并与冠状动脉造影结果相比较,探讨其在冠心病诊断上的优点及不足。方法:48例疑诊冠心病患者,使用双源CT进行冠状动脉成像,了解冠状动脉斑块的种类、大小及解剖学分布,并与冠状动脉造影结果比较,判断其诊断冠心病的敏感性、特异性。结果:48例患者共检查576支冠状动脉节段,检测出127个粥样硬化斑块(约占22%),平均(2.4±0.32)个/例。其中74%为大斑块,26%为小斑块。在所有斑块中,16%的斑块为软斑块,10%的斑块为混合性斑块,74%的斑块为钙化斑块。共检测出8例心肌桥(约占16%)。与冠状动脉造影结果相比较,双源CT诊断冠心病的敏感性、特异性、阳性预测值、阴性预测值分别为91%,97%,97%,98%。结论:双源CT可以实现无创评估冠状动脉的狭窄程度,区分高危斑块,并对心肌桥的诊断具有较高的准确性及敏感性。
