高龄患者脑梗死后认知功能障碍相关危险因素的研究

摘要
目的探讨高龄脑梗死患者认知功能障碍的相关危险因素。方法选取首次发病年龄大于或等于70岁的脑梗死患者300例,采用中文版简易智能量表(MMSE)进行测评,将脑梗死患者分为认知障碍组159例和认知正常组141例,分析两组患者脑梗死后认知功能障碍的发生与人口学资料、血管危险因素、脑梗死特点、脑卒中严重程度的相关性;运用Logistic回归法分析脑梗死后认知障碍的危险因素。结果多因素Logistic回归分析显示低教育程度(OR=1.76,95%CI 1.12~2.28,P<0.01)、脑卒中后抑郁(OR=1.91,95%CI1.31~2.56,P<0.01)、Barthel Index(BI)评分(OR=2.04,95%CI 1.24~2.56,P<0.01)和NIHSS评分(OR=2.16,95%CI1.44~2.79,P<0.01)与脑梗死患者发生认知功能障碍独立相关。结论低教育程度、脑卒中后抑郁和卒中严重程度是高龄脑梗死患者发生认知功能障碍的独立危险因素。
