<正>延续护理(continuing nursing care or transitional nursing)发展初是探索为结束急性期住院治疗的患者提供一种延续性、质量保证的治疗性服务,以促进患者的康复和减少再住院,是一种从医院到家庭/社区不间断地提供的治疗性护理服务,是住院(医院)护理服务的一种延伸,应对的核心是患者在转移期的健康问题和健康需求[1-5];此后延续护理与医院实施疾病诊断治疗分类标准(dignosis-related groups,
