血清维生素D水平与骨代谢状态的相关性:附1389例观察

摘要
目的探讨上海地区人群血清25羟化维生素D水平与甲状旁腺激素及骨代谢指标的关系。方法应用自动电子发光免疫法检测健康成人血清25羟化维生素D(25OHD)、甲状旁腺素(PTH)、骨代谢指标(骨钙素:BGP;Ⅰ型原胶原N端前肽:PINP;β-Ⅰ型胶原C端肽:β-CTX)。按血清25OHD不同水平将受试者分为维生素D极低组、维生素D严重低下组、维生素D低下组、维生素D中间水平组和维生素D高分位5组,比较血清25OHD与甲状旁腺激素及骨代谢的关系。结果接受检测的1 389位受试者年龄为22~88岁,平均年龄(65.8±10.9)岁。男性552名,女性837名,按血清25OHD水平分为5组:极低组≤4 ng/mL,120例(8.6%);严重低下组4.1~10 ng/mL,232例(16.7%);低下组10.1~20 ng/mL,744例(53.5%);中间水平组20.1~30 ng/mL,206例(14.8%);高分位组≥30.1 ng/mL,87例(6.3%)。检测结果显示,血清PTH水平随25OHD升高逐渐降低,呈负相关。血清PTH定值(cut-off value)的平台期在25OHD 16~20 ng/mL之间出现。BGP和PINP在极低组和严重低下组明显增高,差异有统计学意义。矫正PTH后,25OHD与BGP、PINP仍呈负相关。BGP平台期在25OHD 18~25 ng/mL之间出现,PINP平台期在25OHD 20~25 ng/mL之间出现。各组间βCTX差异无统计学意义。结论血清25OHD≤10 ng/mL时,骨形成标志物发生明显变化,血清25OHD是影响骨代谢调节因子之一,但作为单个因子,其影响力度相对较弱。
