脑损伤恢复期偏瘫患者的平衡稳定性

摘要
目的了解偏瘫恢复期患者的平衡能力和姿势对称性。方法偏瘫患者(均具备步行能力)和健康人各24例,采用意大利tecnobody公司生产的PK 254 P平衡反馈训练仪测定患者的平衡能力[压力中心(COP)前后标准差、左右标准差、前后平均运动速度、左右平均运动速度、运动长度、运动椭圆面积]和对称性(X轴平均COP),分别在标准静态坐位及站立位下测试30 s。结果偏瘫患者在静态坐位和站立位下的COP前后标准差、左右标准差、前后平均运动速度、左右平均运动速度、运动长度、运动椭圆面积均高于正常对照组(P<0.05),偏瘫患者的X轴平均COP大于正常对照组(P<0.05)。X轴平均COP与反应平衡能力的6个参数之间具有一定的相关性(r=0.44~0.68)。结论 PK 254 P平衡反馈训练仪对偏瘫患者坐位和站立位下的平衡能力的检测具有很好的敏感性。偏瘫患者即使具备步行能力,但其躯干控制能力与站立位下的平衡能力较差,所以对具备步行能力的偏瘫患者仍需进行平衡训练以进一步提高其生活质量和行走能力。静态站立位下偏瘫患者体质量分布不对称,其重心仍偏向健侧;偏瘫患者的体重分布不对称性越严重,平衡能力越差。
