基于“筋出槽、骨错缝”理论探讨神经根型颈椎病的CT特征

摘要
目的:拟利用三维CT重建颈椎的空间形态,全方位了解神经根型颈椎病患者"筋出槽、骨错缝"的影像学特点,为神经根型颈椎病"筋出槽、骨错缝"的临床特征提供影像学依据。方法:纳入30例C5-6椎间盘单侧突出的神经根型颈椎病患者的CT资料,通过在矢状位测量C5-6及C6-7颈椎间盘高度和颈椎间盘前后缘高度差、在冠状位测量C5-6颈椎间盘左右两侧高度和钩椎关节间隙及在轴位测量C5-6左右两侧关节突关节矢状角角度和关节突关节间隙高度,从而了解C5-6神经根型颈椎病患者颈椎"筋出槽、骨错缝"的影像学特点。结果:在矢状位颈椎CT上,C5-6颈椎间隙下沉,并且C5椎体前屈;在冠状位颈椎CT上,C5-6节段没有发生明显侧屈,并且颈椎间盘突出的方向与C5-6节段椎体侧屈方向无相关性;在轴位颈椎CT上,C5-6颈椎间盘突出方向倾向于关节突关节矢状角角度较大的一侧,颈椎间盘突出方向倾向于小关节退变程度较轻的一侧,并且推测C5椎体向健侧旋转。结论:C5-6神经根型颈椎病患者的CT特征是:C5-6颈椎间隙下沉、C5椎体前屈并向健侧旋转。
