基质金属蛋白酶2在胆道闭锁患儿肝组织中的表达

摘要
目的研究胆道闭锁(BA)患儿肝组织中基质金属蛋白酶2(MMP-2)与肝纤维化、Kasai术后黄疸清除、胆管炎发作和自体肝生存时间的关系。方法收集广西医科大学第一附属医院2009年1月至2010年12月行Kasai手术并已获得2年随访的65例BA患儿(BA组)的临床和病理资料, 并取同期15例无肝胆系统疾病患儿(对照组)肝组织进行对照。免疫组织化学法测定肝组织MMP-2的表达。结果肝纤维化Ohkuma''s分级BA组高于对照组(Z=-4.02, P<0.001);MMP-2染色阳性率BA组高于对照组(χ2=33.88, P<0.001), MMP-2表达强度BA组高于对照组(Z=-5.14, P<0.001);MMP-2表达随肝纤维化Ohkuma''s分级增高而增强(r=0.686, P<0.001), 随手术年龄增加而增强(r=0.465, P<0.001);MMP-2表达强度越高, Kasai术后黄疸完全消退率降低(r=-0.687, P<0.001), 胆管炎发作率增高(r=0.361, P=0.003);MMP-2表达强度较低者Kasai术后2年自体肝生存率升高(χ2=64.000, P<0.001)。结论 BA患儿肝组织MMP-2的表达与肝纤维化程度、Kasai术后黄疸清除、胆管炎发作和自体肝生存时间关系密切, 是评价Kasai术后治疗效果和预后的参考指标。
