下颈椎经关节螺钉固定的研究进展

<正>1987年,Magerl等首次运用经关节螺钉(transarticular screw,TAS)固定寰枢关节。这一固定方法虽然在生物力学上明显优于其他方法,但由于C1/2复杂的解剖关系,因此对技术要求很高,且椎动脉损伤的风险大。然而,2003年,Takayasu等在25例患者中应用下颈椎TAS固定,共置入81枚螺钉,没有出现任何并发症。经过3个月～5年的随访,无一例螺钉松动或脱出,且都成功融合。笔者认为,下颈椎TAS钉道穿过上、下关节突四层皮质骨,具有良好的生物力学稳定性,而且这一技术相对安全、简易,值得临床推广。
