摘要：

当前中国处于人口健康转型阶段,慢性病主要危险因素处于失控状况,潜在危险日益严重,疾病的直接、间接负担呈明显的加速趋势,健康形式不容乐观。推进医疗、预防、保健的整合,从保健政策上支持医学整合,是控制危险因素,遏止、扭转和减少慢性病的蔓延非常关键性的措施。
