力线标记物后置法在胫骨高位截骨术中的应用附视频

摘要
目的观察并评价力线标记物后置法在胫骨高位截骨术中的应用效果。方法纳入自2013-02—2017-02诊治的56例(62膝)膝关节内侧间室骨性关节炎,采用胫骨高位开放楔形截骨矫形内固定术治疗。20例(22膝)术中采用常规力线标记物前置法定位下肢机械力线(常规组),36例(40膝)术中采用力线标记物后置法定位下肢机械力线(改良组)。比较2组手术时间、术中X线透视次数,术后即刻及末次随访时HKA角,以及末次随访时HSS评分、WOMAC评分。结果56例均获得随访,随访时间平均35.8(12~60)个月。所有患者切口均一期愈合,未出现感染及下肢深静脉血栓形成等并发症。与常规组比较,改良组手术时间更短,术中X线透视次数明显更少,差异有统计学意义(P <0.05);但2组术后HKA角、HSS评分、WOMAC评分比较差异无统计学意义(P>0.05)。结论胫骨高位截骨术中采用力线标记物后置法可明显缩短手术时间和减少X线透视次数,无需其他设备辅助,简便易行,值得临床推广应用。
