经皮椎间孔镜治疗腰椎间盘突出症的临床效果

摘要
目的:探讨经皮椎间孔镜(percutaneous transforaminal endoscopic discectomy,PTED)治疗腰椎间盘突出症的手术方式、手术技巧及其临床效果。方法:回顾2010年7月至2014年7月在我院行椎间孔镜治疗的腰椎间盘突出症患者300例,其中男182例,女118例;年龄16~82岁,平均45岁。所有患者均经腰椎动力位片证实无手术节段腰椎失稳。手术方式采用PTED技术中的YESS技术137节段,TESSYS技术246节段。采用视觉模拟评分法(VAS法)和Macnab疗效评定标准对患者入院时和椎间孔镜术后当时、3月及1年进行评分,评估经皮椎间孔镜技术治疗腰椎间盘突出症的疗效。结果:其中120例患者获得1年的随访。术后当时、3个月及1年的VAS评分分别为1.74±0.76、1.65±0.74及1.56±0.77,均显著低于术前的8.57±0.71(P<0.05),差异有统计学意义。根据Macnab疗效评定优92例,良17例,可8例,差3例,术后优良率均为90.8%。结论:经皮椎间孔镜在多方面具有微创优越性,是治疗腰椎间盘突出症的一种安全、有效的方法。
