富含支链氨基酸和谷氨酰胺的营养配方对贲门癌术后早期机体免疫功能的影响

摘要
目的:探讨富含支链氨基酸和谷氨酰胺的肠外营养(PN)配方对贲门癌患者术后机体免疫功能的影响。方法:选择贲门癌行根治性全胃/近端胃切除术患者54例,随机分成两组,两组患者均从术后第1日分别接受等热量、等氮量的常规PN(对照组)和富含支链氨基酸和谷氨酰胺的PN(研究组)支持治疗,并于术晨、术后第1、8天测定血液C3、IgA、IgM、IgG、TNF-α、IL-6水平和CD3+、CD4+、CD8+、CD4+/CD8+、NK细胞百分率,并检测红细胞免疫功能。结果:术后8d后研究组血清补体C3、IgA、IgG、CD4+、CD4+/CD8+、NK细胞的恢复程度与对照组差异有显著性。红细胞C3b受体花环率(RCR)和肿瘤红细胞花环率(TRR)研究组患者的红细胞免疫功能高于对照组,差异有统计学意义。而两组间血清TNF-α、IL-6水平并无明显差异。结论:与常规PN对比,富含支链氨基酸和谷氨酰胺的强化PN能改善贲门癌患者术后免疫功能。
