初次全髋关节置换术治疗老年股骨颈骨折的近期疗效及影响因素分析

摘要：

目的探讨初次全髋关节置换术(THA)治疗老年股骨颈骨折的近期疗效及影响因素。方法回顾性分析2013年1月至2015年5月期间初次采用THA治疗的116例老年股骨颈骨折患者资料。男34例, 女82例;平均年龄为70.2岁。骨折按Garden分型:Ⅰ型23例, Ⅱ型34例, Ⅲ型38例, Ⅳ型21例。统计患者的性别、年龄、体重指数(BMI)、致伤原因、术前合并内科疾病、骨折Garden分型、手术参数、术后功能锻炼时间及术后并发症等资料。先采用χ2检验对上述各相关因素进行单因素分析, 再采用多因素logistic回归分析筛选影响髋关节功能的主要因素。结果 116例患者术后获12～23个月(平均18.3个月)随访。末次随访时按髋关节Harris评分评定髋关节功能:优36例, 良52例, 可20例, 差8例, 优良率为75.9%。单因素分析结果显示:年龄、BMI、手术时间、手术入路、双下肢长度差、术后是否镇痛、总失血量、术后功能锻炼时间、术后并发症与术后髋关节功能的恢复有关, 差异有统计学意义(P<0.05)。多因素logistic回归分析结果显示:BMI、手术入路、双下肢长度差、术后是否镇痛、总失血量、术后功能锻炼时间是影响髋关节功能恢复的主要因素(P<0.05)。结论初次THA治疗老年股骨颈骨折疗效良好。BMI偏小、选择微创入路、缩短双下肢长度差、术后镇痛、减少总失血量及延长术后功能锻炼时间等都有利于患者THA术后髋关节功能的恢复。
