吸入丙酸氟替卡松治疗儿童哮喘疗效观察

目的观察吸入丙酸氟替卡松治疗儿童哮喘的疗效及副作用。方法 304例哮喘患儿根据病情严重程度予吸入丙酸氟替卡松125μg/d、250μg/d和375μg/d,分别于初诊时及治疗2、4和12周后,评价其日、夜间哮喘症状计分和变应性鼻炎计分,记录吸入β2受体激动剂(喘乐宁等)的揿数和应用天数,听诊哮鸣音改善情况。其中对266例于初诊时和治疗12周后监测最高呼气流速(PEFR)值或第1秒用力呼气量(FEV1)值。结果经吸入丙酸氟替卡松治疗2、4和12周后与初诊基础值比较,患儿日、夜间哮喘症状计分、变应性鼻炎计分和吸入β2受体激动剂揿数均有显著性差异(P<0.01),并且这些作用随着治疗时间的延长更加明显;第12周时PEFR值占预计值的百分比或FMV1值占预计值的百分比和初诊时相比较有显著性差异(P<0.01),吸入β2受体激动剂天数明显减少,哮鸣音均逐渐减少至消失,且无明显副作用。结论吸入丙酸氟替卡松治疗不同严重程度的儿童哮喘,疗效好,而且安全。
