肿瘤坏死因子受体1、2与糖尿病肾病关系的研究进展

摘要
近年来越来越多的研究发现慢性低度炎症在糖尿病肾病的发病机制中起重要作用,肿瘤坏死因子受体1(TNFR1)和肿瘤坏死因子受体2(TNFR2)是主要的炎症因子,在动物实验中发现肾小球的炎症反应调节大部分是通过TNFR1依赖的mRNA介导的促炎反应,对糖尿病肾病鼠肾组织进行免疫组织化学染色发现TNFR1阳性细胞在近端肾小管有大量聚集。研究发现TNFR1、TNFR2在1型糖尿病早期肾功能下降时就有升高,而尿蛋白阴性但TNFR1、TNFR2浓度较高的2型糖尿病病人发生终末期肾病的风险明显升高。提示TNFR1、TNFR2或可为糖尿病肾病的诊断提供新的依据。
