急性缺血性脑卒中患者重组组织型纤溶酶原激活剂静脉溶栓治疗后的血压与预后的关系

摘要：

目的探讨重组组织型纤溶酶原激活剂(rt-PA)静脉溶栓治疗急性缺血性脑卒中患者后血压及血压变化与预后的关系。方法回顾性分析接受rt-PA静脉溶栓治疗的急性缺血性脑卒中患者137例的资料,按治疗后第90天改良Rankin量表(mRS)评分分为预后良好组(0～1分)及预后不良组(2～6分),比较这两组患者溶栓治疗后24h血压及血压变化情况。结果预后不良组与预后良好组在溶栓前基线血压水平差异无统计学意义。溶栓治疗后2和24h,预后不良组收缩压分别为(147.4±18.5)和(147.7±17.7)mm Hg,预后良好组分别为(140.9±17.5)和(135.9±21.1)mm Hg;溶栓后24h预后不良组收缩压降低幅度小于预后良好组[(-0.54±18.02)比(9.60±17.33)mm Hg,P<0.05]。多因素Logistic回归分析结果显示,溶栓前美国国立卫生研究院脑卒中量表评分、既往有糖尿病、溶栓前血糖、溶栓后2h及24h收缩压、溶栓后24h收缩压变化与溶栓预后独立相关。结论溶栓后2h及24h收缩压较低者的溶栓预后好。
