基于退变腰椎间盘模型的旋转手法对椎体角度位移的影响

摘要
目的:在模拟椎间盘退变基础上,分析卧位旋转手法时腰椎体角度位移变化,探讨旋转手法对椎间盘的力学作用机制。方法:通过Motion运动捕捉系统采集12具腰椎间盘病理状态椎体左右旋转屈伸、侧弯角度位移。结果:1)右旋转各间盘组内屈伸、侧弯、旋转角度位移差异无统计学意义(P>0.05),轴向旋转角度位移有小于屈伸、侧弯角度位移的趋势。旋转角度位移L3>L4>L5,侧弯角度位移L3>L4>L5.2)左旋转L5椎体角度位移旋转小于屈伸,差异有统计学意义(P<0.05);其余各间盘组内屈伸、侧弯、旋转角度位移差异无统计学意义(P>0.05)。3)L3椎体在左右旋转时,侧弯角度位移差异有统计学意义(P<0.05),左旋转L3椎体侧弯角度位移小于右旋转,其余方向的角度位移差异无统计学意义(P>0.05);L4,L5椎体在左右旋转时,屈伸、侧弯、旋转角度位移在三种方向的变化差异无统计学意义(P>0.05)。结论:1)旋转手法治疗腰椎间盘突出症的力学机制是脊柱的共轭运动。2)旋转手法对不同椎体作用力的效果不一致。3)侧转运动的力学作用机制应进一步研究。
