脑梗死患者梗死部位和数量与认知功能损害的相关性研究

摘要
目的探讨脑梗死患者脑梗死部位和数量与预测认知功能损害的关联性。方法应用神经心理学量表检测复合记忆、加工速度、执行功能, 结合应用磁共振影像确定皮层, 皮层下和小脑梗死, 研究脑梗死的部位和数量与认知成绩之间局部交叉的相关性。结果只有皮层或只有皮层下梗死者与对照组比较, 复合记忆成绩显著较差[AVLT-5分别为(6.14±1.89)分及(7.26±1.82)分, P<0.01];对于加工速度, 皮层和皮层下受损的联合效应大于只有皮层或皮层下梗死者;多部位的多发性梗死与对照组比较表现出复合记忆[LMT即刻分别为(11.12±2.73)分及(13.40±2.29)分, P<0.01]、加工速度[SDMT分别为(27.08±11.13)分及(34.23±7.44)分, P<0.01]或执行功能[TMT-B错误数分别为(1.34±0.65)个及(0.40±0.35)个, P<0.01]3个认知域成绩的下降。结论脑梗死患者多部位的梗死与较差的复合记忆、加工速度或执行功能有关, 提示梗死的数目和部位共同导致了认知损害。
