桥本甲状腺炎患者外周血调节性B淋巴细胞和抑制性T细胞亚群的免疫改变

摘要
目的探讨桥本甲状腺炎(HT)患者外周血调节性B淋巴细胞(regulatory B cell, Bregs)、抑制性T细胞(suppressor T cell, Ts)的比例变化。方法入组18~60岁桥本甲状腺炎患者61例, 性别年龄相匹配的健康组38名。应用流式细胞技术检测外周血CD19+CD24+CD27+B细胞、CD19+CD24hiCD38hiB细胞、CD19+白细胞介素-10+B细胞及CD8+CD28-T细胞比例变化。结果与对照组相比, HT组CD19+CD24+CD27+B细胞亚群显著升高(P<0.05)。随着甲状腺过氧化物酶抗体的升高, CD19+CD24hiCD38hiB细胞显著降低(P<0.05);随着甲状腺球蛋白抗体(TGAb)的升高CD8+CD28-T细胞显著降低(P<0.05)。三碘甲状腺原氨酸(TT3)与CD19+CD24hiCD38hiB细胞则呈显著正相关;甲状腺素(TT4)与CD8+CD28-T细胞呈显著负相关, 与CD19+CD24+CD27+B细胞则呈显著正相关(P<0.05)。Logistic回归分析发现, TGAb和游离甲状腺素(FT4)是CD8+CD28-T细胞在淋巴细胞中比例降低的独立危险因素(P<0.05);促甲状腺素(TSH)是CD19+CD24hiCD38hiB细胞比例降低的独立危险因素, FT4是CD19+CD24+CD27+B细胞比例升高的独立危险因素(P<0.05)。结论 HT患者外周血Ts显著降低, 不同表型的Bregs比例变化则不同, 甲状腺自身抗体和甲状腺功能均与HT的免疫改变相关。
