顺势互动复位技术治疗难复性股骨颈骨折

摘要
目的探讨应用顺势互动复位技术治疗难复位性股骨颈骨折的效果。方法采用回顾性病例对照研究分析2011年1月— 2014年1月收治的91例难复性股骨颈骨折患者临床资料。按照治疗方法将患者分为研究组和对照组。其中研究组47例, 男31例, 女16例;年龄23~61岁, 平均40.6岁。对照组44例, 男30例, 女14例;年龄28~63岁, 平均43.1岁。骨折类型均为Garden Ⅳ型。研究组采用顺势互动复位技术治疗, 对照组采用闭合复位辅助有限切开内固定治疗。两组均采用3枚直径6.5 mm空心加压螺钉固定。记录手术时间、术中出血量。术后摄患侧髋关节正、侧位X线片, 根据Garden指数评估骨折复位固定质量。随访观察骨折愈合情况、股骨头坏死等并发症。末次随访时根据髋关节Harris评分评估患髋关节功能。结果研究组手术时间为(41.2±7.5)min, 对照组为(105.0±15.7)min;研究组术中出血量为(37.3±9.5)ml, 对照组为(269.6±50.6)ml(P<0.05)。研究组术后随访时间为(4.0±0.9)年, 对照组为(3.8±0.8)年(P<0.05)。两组复位质量比较, 研究组Garden指数Ⅰ级44例, Ⅱ级3例;对照组Ⅰ级20例, Ⅱ级24例(P<0.05)。研究组术后出现股骨头坏死4例, 对照组10例(P>0.05)。研究组骨折均愈合, 对照组发生骨折不愈合8例(P<0.05)。研究组Harris评分优39例, 良4例, 可4例, 优良率为91%;对照组优21例, 良8例, 可15例, 优良率为66%(P<0.05)。结论顺势互动复位技术以近端对远端的方式实现股骨颈骨折的闭合复位, 与闭合复位辅助有限切开内固定治疗比较, 可缩短手术时间, 减少术中出血量, 提高术后复位质量, 降低术后骨折不愈合和股骨头坏死的发生率。
