血清胱抑素与血肌酐监测在心脏手术相关急性肾损伤诊断中的应用

摘要
目的探讨血清胱抑素(Cystatin C)和血清肌酐(SCr)监测在心脏手术相关急性肾损伤(CSA-AKI)诊断中的应用价值。方法收集南方医院胸心外科行心脏瓣膜置换术的患者90例,在术前及术后不同时间点测定血清Cystatin C、SCr,观察其在术后不同时间点的动态变化;运用接受者工作特征曲线(AUC-ROC)评价其对早期监测CSA-AKI的价值。结果术后2 h,AKI组患者的SCr水平较术前显著升高(P<0.01),SCr诊断AKI的ROC曲线下面积为0.75;术后第1天,SCr诊断CSA-AKI的ROC曲线下面积为0.90,取101.5μmol/L时,SCr诊断CSA-AKI的敏感度0.81,特异度0.83;此后,SCr水平虽持续在峰值但诊断效能逐渐下降,术后第3天其ROC曲线下面积为0.81。术后第1天,血清Cystatin C较术前显著升高(P<0.01),其诊断CSA-AKI的ROC曲线下面积为0.70。此后血清Cystatin C水平及诊断效能均进行性升高,术后第3天,血清Cystatin C诊断CSA-AKI的ROC曲线下面积为0.83,取1.29 mg/L时其诊断敏感度0.71,特异度0.86。结论血清Cystatin C早期诊断CSA-AKI的价值有限,其对监测AKI的进展及持续状态更有意义;而SCr对心瓣膜置换术后急性肾损伤的早期诊断更为敏感,术后第1天的SCr水平对于诊断CSA-AKI有重要意义。
