电视胸腔镜与开胸手术治疗非小细胞肺癌随机对照研究的系统评价和Meta分析

摘要
目的对电视胸腔镜与开胸手术治疗非小细胞肺癌进行系统评价,探讨两组手术的治疗效果。方法计算机检索EMbase、PubMed、OVID、Springer Link、Cochrane Library、CNKI、CBMdisc、VIP等数据库,检索时间为每个数据库建库至2014年10月。收集有关胸腔镜和开胸手术治疗非小细胞肺癌的随机对照研究(RCT)。由两名研究者对文献质量独立进行评价并提取相关数据,采用Rev Man5.3软件进行Meta分析。结果共纳入10篇RCT,患者1 529例。其中电视胸腔镜组(VATS)453例,开胸组1 076例。Meta分析结果显示:两组手术在淋巴结清扫数目(P=0.41)、手术时间(P=0.09)、术中出血量(P=0.14)、胸腔引流管留置时间(P=0.53)、手术死亡率(P=0.72)、总体生存率(P=0.39)方面差异无统计学意义;两组胸腔引流量(P=0.04)、术后住院时间(P=0.01)、并发症发生率(P=0.0001)差异有统计学意义。结论电视胸腔镜手术治疗非小细胞肺癌安全、有效。
