Traditionally, the pathogenesis of chronic rhinosinusitis was attributed to the persistence of bacterial infection and inflammation after acute sinusitis. Evidence suggests that immunologic stimuli, such as fungal antigens or bacterial superantigens, may lead to chronic inflammation of the paranasal sinuses. Research into the immune mechanisms of chronic rhinosinusitis is in its early stages. This article summarizes the understanding and controversies regarding the mechanism of chronic rhinosinusitis and identifies issues that need to be addressed by future studies.
