Changes in thyroid function in the setting of systemic illness, surgery, or fasting are referred to as nonthyroidal illness syndrome (also called "sick euthyroid syndrome"). Nonthyroidal illness influences thyroid hormone production and action at multiple levels including the hypothalamic-pituitary-thyroid axis, thyroid hormone transport, and metabolism. This article reviews the pathophysiology of these changes and the influence on interpretation of thyroid function tests. Guidelines are provided for the assessment of thyroid function in sick patients and the options for treatment.
