Safety does not happen by accident, it is managed and a vast array of factors are taken into account during this process. It is also useful to learn from others' experiences. The shared experience of the recent FSF/IFA/IATA symposium* was safety management and, as STEWART PENNEY reports, a safety conscious organisation has to take many factors into account.
