The diagnosis and differential diagnosis of endogenous Cushing's syndrome are among the most challenging problems in clinical endocrinology. Traditional approaches have failed to provide adequate sensitivity, specificity, and accuracy for the diagnosis of Cushing's disease. This article contrasts newer diagnostic techniques with traditional approaches. New problems in the evaluation and management of patients with suspected Cushing's disease also are reviewed.
