Today, we know that most acromioclavicular joint separations may be treated nonoperatively. We have arrived at this conclusion through a tortuous route of both operative and nonoperative interventions, however. This article reviews the literature that has helped us in our current approach to managing acromioclavicular joint separations and introduces the management options for the various types of injuries.
