Pathologic parameters in the diagnosis and prognosis of primary cutaneous melanoma

Significant histopathologic, immunohistochemical, and investigative studies conducted in the last 10 years and relevant to the diagnosis and prognosis of primary cutaneous melanoma are reviewed. Important recent concordance studies in the diagnosis of melanoma are discussed and the relevant histologic prognostic indicators, such as thickness, mitotic rate, number of tumor-infiltrating lymphocytes, regression, and ulceration, are summarized. The value of classifying melanoma into nontumorigenic radial growth and tumorigenic vertical growth phases as a model to predict outcome is contrasted with subclassification. Finally, a critical appraisal of established and emerging immunohistochemical markers for the diagnosis and prognosis of melanoma is presented.
